Diagnosis and Intensive Care for Functional Intestinal Obstruction

Abstract


The article presents the modern methods of diagnostics and intensive therapy for functional intestinal obstruction. Emphasis is placed on the use of multimodal approach to conservative treatment of paresis and intestinal atony. It primary includes decreased usage of drugs that inhibit the motility (narcotic analgesics, excessive fluid therapy), correction of fluid and electrolyte disorders, as well as early enteral nutrition. On the basis of this approach, the use of prokinetics, which are described in detail in our work, becomes much more efficient. When selecting drugs to stimulate the intestinal peristalsis, polypharmacy has to be avoided, and this publication presents most authoritative literature data about the usage of prokinetic and their generalization. The paper also shows the results of our researches of drugs with low affinity for the mu-opioid receptor. In addition, we describe the experience of our own clinical observations, in terms of both short- and long-term use of these drugs in patients with various forms of functional intestinal obstruction in Kyiv since 2011.
