Fever in Acute Stroke: Problems of Diagnosis and Treatment

Abstract


The article presents a review of literature devoted to the problem of fever in patients with acute stroke (AS). High body temperature was noted to have a significant correlation with stroke severity, lesion size, mortality and neurological outcomes. In patients with AS in the differential diagnosis of fever it is of great importance time of its occurrence, clinical features and the C-reactive protein level. Increased body temperature in stroke patients must always be lowered and maintained in a safe normothermic range at least during the first few days after the stroke start. Acetaminophen for the treatment of fever has limited effectiveness in patients with stroke. To avoid the side effects daily acetaminophen dose should not exceed the new standard dose — 3 g/day. More researches are needed to find out whether the fever is one of the pathogenetic mechanisms or just a marker of adverse outcome, and to determine the optimal treatment program.
