Infusion-Transfusion Therapy (Post-Transfusion Complications, Homologous Blood Syndrome) Lecture for Medical Interns

Abstract


Complications of infusion-transfusion therapy may arise both during transfusion of the blood and plasma substitutes, and after a while. Complications more often develop after repeated, long-term and large-volume transfusions. They can be caused by incorrect determination of long-term storage of blood, infection of transfusion media, violation of infusion technology, allergic reactions of the body, thrombosis, phlebitis. Besides, the transfusion of blood or its components from a donor who had serum hepatitis can result in infection of recipient.
Allergic and pyrogenic post-transfusion complications can occur in transfusion of any infusion medium, most often in patients undergoing long-term and massive infusion therapy. However, there are cases when most severe reactions, ended in the death of patients, occur at the first introduction of a solution, for example haemodez. Most allergic complications develop during the transfusion of protein plasma substitutes, amino acid solutions and fat emulsions. They usually occur acute, sometimes after administration of 50–100 ml of the solution.
To prevent the development of massive transfusion syndrome, it is necessary to carry out a number of preventive measures, and strictly observe the basic principles of transfusiology.
