Clinical and Immunological Aspects of Pharmacological Cardioversion in Patients with Cognitive Dysfunction under Atrial Fibrillation

The purpose of this study was to investigate the clinical and immunological status of the patients with cognitive dysfunction under atrial fibrillation and efficacy of treatment with amiodarone in those patients. The patients were divided into two groups: the I group consisted of 46 patients who were undegone pharmacological cardioversion with amiodarone; the ІІ group included 32 patients in whom sinus rhythm was not restored due to the presence of contraindications. There were studied the level of fatigue, results of MMSE test, Schulte tables, clock-drawing test, the battery of tests to assess frontal lobe dysfunction, verbal associations (regular and categorical), samples memorizing 12 words with direct reproduction. The levels of proinflammatory cytokines (IL-6, TNF-α) and CRP were determined by enzyme-linked immunosorbent assay. After treatment the patients of the І group had better neurocognitive functions compared with the patients of the ІІ group. Reduced levels of proinflammatory cytokines in patients of І group was statistically significant. Pharmacological cardioversion with amiodarone significantly effect the neuropsychological parameters, improving the condition of the patients with atrial fibrillation and their quality of life. Restoration of sinus rhythm in the patients with atrial fibrillation is accompanied by decreased activity of systemic inflammation.
