Prognostic significance of the level of galectin-3 in patients with hypertrophic cardiomyopathy

Abstract


The aim of the study was to investigate the prognostic value of the level of galectin 3 (Gal-3) in patients with hypertrophic cardiomyopathy (HCM). Materials and methods. The clinical-anamnestic and instrumental data were studied in 90 patients with HCM, of which 76 were examined again after 12 ± 3 months of treatment. Methods — electrocardiography (ECG), echocardiography, ECG monitoring, and evaluating the level of Gal-3 in the blood serum by enzyme-linked immunosorbent assay. Results. Re-examination of clinical and instrumental research data and level of Gal-3 in the blood serum of patients with HCM 12 ± 3 months after initial examination made it possible to identify two groups of patients: with a stable and progressive course of the disease. The progressive course of HCM often observed in its obstructive form, the presence of concomitant coronary artery disease, hypertension, persistent atrial fibrillation and severe heart failure. From the data of instrumental investigation, the most informative were echocardiography indicators, which revealed that at the progressive course of HCM, the systolic and diastolic left ventricular dysfunction developed. However, the most significant correlation with the progressive course of HCM had initial increase of the Gal-3 level, indicating the prognostic value of this indicator in patients with HCM. The final regression model prognosis of HCM was included heart failure severity, level of Gal-3, and echocardiography indicators, characterizing the systolic and diastolic left ventricular function. The effectiveness of the proposed model for HCM prognosis was 90.8 %, which indicates the possibility of its practical application. Conclusions. The initial increase of Gal-3 in patients with HCM is associated with a progressive course of the disease; the assessment of its level in conjunction with clinical and instrumental parameters is sui­table for predicting the course of HCM.
