Three-year experience in implementing the OSCE for intermediate certification of interns specialized in the anesthesiology and intensive care

The modern level of medicine development requires of an anesthesiologist to master a large volume of theoretical knowledge and practical skills, the ability to use them in complex and critical conditions. Proceeding from this, the urgent need to optimize the educational process requires restructuring the systems of assessment and control of acquired knowledge and practical skills. Three years ago, an objective structured clinical examination (OSСE) was introduced at the department of anesthesiology, intensive care and medicine of emergency conditions of the postgraduate education faculty for the intermediate control of theoretical knowledge and practical skills of the first-year interns. The purpose of this work is to analyze the results of an objective structured clinical examination and to assess the intermediate knowledge and practical skills of the first-year interns specialized in the anesthesiology and intensive care in accordance with the OSCE methodology. For the examination, 7 stations were prepared: clinical stations, which included the interaction of interns with a real or standardized patient; practical stations (performing technical procedures and manipulations) and static (theoretical) “stations”. The assessment tools were valuation sheets and global rating scale adapted to the professional requirements. Of the 32 interns who passed the exam, only 10 received more than half of the possible number of points, the rest — less. The average score was 10.3 (out of 14 possible). Most interns, whom we thought they would successfully pass this exam, hardly exceeded the figure of 60 %. The highest scores were received by interns for preoperative preparation of patients, blood transfusions, as well as demonstrations of resuscitation skills on the manikin. Interns had low scores for answers about intravenous anesthesia and for a practical skill in placing a laryngeal mask. In the future, we plan to develop additional stations, according to the requirements of the standard curriculum for internships of the Ministry of Health of Ukraine, to improve the methodological support for each new station, to adapt the criteria for assessing practical skills and to introduce it for the final certification for the title of medical specialist.Downloads
