The case report of catastrophic antiphospholipid syndrome

The case report of the patient with thrombophilia and thrombocytopenia is presented. A 75-year-old man had clinical manifestations of catastrophic antiphospholipid syndrome as multifocal venous and arterial thrombosis which lead to multiple organ dysfunction (renal, hepatic, cerebral and cardiovascular), distal necrosis in all extremities, thrombocytopenia. The treatment with anticoagulant, antiplatelet agents and corticosteroid therapy, plasma exchange led to improvement of patient’s state. Due to distal necrosis, fingers on both hands were amputated, and the right lower limb — at the level of the middle third. The literature data and clinical guidelines are analyzed and variants of diseases, which can be possible causing factors of thrombophilia with thrombocytopenia, are discussed. The most difficult points in the management of such patients are described.
