The impact of modified infusion therapy and adrenomimetic correction on the parameters of oxygen transport, consumption, indicators of peripheral circulation at multiple organ failure syndrome in patients with severe polytrauma

Abstract

The article deals with the investigation of influence of the modified infusion therapy and adrenomimetic support on the parameters of oxygen transport (DO2), consumption (VO2), peripheral circulation. It was estimated that applied protocol of modified infusion therapy accor­ding to the stage of postinjury period preserves DO2 above 550 ml/min/m2; VO2 more than 150 ml/min/m2; reestablishes the levels of lactate in arterial blood, normalizes the gradients of PvCO2-PaCO2, Tperif-Tatm, Tcentr-Tperif, Tskin diff, preserves the urine output more than 1 ml/kg/h. The applied protocol of modified infusion therapy and adrenomimetic support according to the stage of postinjury period preserves the level of DO2 more than 600 ml/min/m2; the venous saturation rate more than 70 %; decreases the level of lactate in arterial blood; increases the levels of BE. Our results also confirm that the applied protocols provided an improvement of peripheral circulation: this statement was confirmed by the increase of Tperif-Tatm gradient and urine output and decrease of gradients PvCO2-PaCO2; Tcentr-Tperif; Tskin diff.
